Semaglutide is a long-acting glucagon-like peptide-1 (GLP-1) receptor agonist. Structural modifications (an Aib substitution and a fatty-acid side chain) confer resistance to DPP-4 degradation and binding to serum albumin, extending its half-life to allow weekly dosing.
It has been studied in extensive clinical trial programs for glycemic control in type 2 diabetes (SUSTAIN trials) and for weight reduction in adults with obesity or overweight (STEP trials). More recently it has been investigated for cardiovascular outcomes (SELECT) and other metabolic endpoints.
It is used as a reference GLP-1 receptor agonist in metabolic research.
